Confirm Eligibility

Confirm that your organization falls within the scope of PacificSource Foundation for Health Improvement funding:

  • Healthcare Access Grantees must be qualified as a tax-exempt organization under IRC 501(c)(3)
  • Strategic Grantees must meet the IRS’s requirements for a tax-exempt organization.
  • Your organization’s primary focus must be community health, healthcare and/or access to healthcare within the community.
  • Your grant proposal must benefit people in the communities served by PacificSource Health Plans.
  • The Community Health Excellence (CHE) Grant Program has additional criteria. For more information about the CHE Grant Program, click here.

To assure PacificSource Foundation for Health Improvement contributions directly support the health and welfare of the underserved, we do not fund capital campaigns or requests that are exclusively for the purchase of equipment.  

We do fund indirect costs up to 10 percent of the requested budget. Examples of indirect costs include utilities, rent, administrative staff, audit, and evaluation.

Submit a Letter of Intent

All interested applicants must first submit a brief Letter of Intent (LOI). LOIs will be reviewed for strength of alignment with foundation priority areas, strength of proposal and available resources. You will receive confirmation when your LOI is received.
