Voluntary Student Eligibility and Cost

Sign up for coverage: To enroll in voluntary student coverage, you will need to complete an enrollment form. 

Domestic students are eligible for the Oregon State University Student Health Insurance Plan on a voluntary basis if they are an enrolled student at Oregon State University (six credit hours for undergraduate students; three credit hours for graduate students).

E-campus students with the following criteria have the option to enroll in the OSU Student Health Insurance Plan:

  • Student must be degree-seeking. 
  • Undergraduate students must take a minimum of six credits.
  • Graduate students must take a minimum of three credits.
  • Student must be living on campus or within designated ZIP codes surrounding the Corvallis campus.

PharmD students are eligible for the OSU Student Health Insurance Plan on a voluntary basis.

Postdoctoral Fellows, Vet Residents, and Visiting Scholars are eligible for the OSU Student Health Insurance Plan on a voluntary basis.

You will need to complete an enrollment form. Payment instructions will be provided by PacificSource, once your enrollment has been processed.

Eligible dependents of those enrolled in the plan may participate on a voluntary basis.

Paying for coverage:

  • Mail — Complete a form and mail it to us with your check or money order. 
  • Email and phone — Complete a form and email it to membershipstudentreps@pacificsource.com. Allow 3–5 business days for processing, then call toll-free (866) 999-5583 ext. 6809 to make your payment over the phone via credit card, or you can wait until you receive information from us on how to pay.

Online Account Access through InTouch

After your coverage begins, you can set up access to your PacificSource plan information through our secure website, InTouch for Members. Once you've signed up, you can log in to see your PacificSource account information and access online tools such as CaféWell. And you can sign up to receive explanation of benefits statements by email, instead of mail.

When you visit your doctor or pharmacy, you'll need to present your PacificSource member ID card to ensure they have the correct insurance information. You can print an ID card (or use the myPacificSource app) as soon as your coverage begins.

Your PacificSource coverage also includes the following no-cost wellness programs and services. (These are not insurance.)

  • 24-Hour NurseLine. Have a health-related question? Call our 24-Hour NurseLine. Staffed around the clock, seven days a week, you’ll always have a registered nurse to talk to. Call toll free (855) 834-6150.
  • Virtual healthcare visits with Teladoc®. Teladoc is a national network of U.S. board-certified physicians and pediatricians that you can see on-demand, 24/7, via phone or online video consultations, from wherever you happen to be; some limitations apply.
  • Assist America® Global Emergency Services. If you experience a medical emergency 100 or more miles from home, Assist America is on call to coordinate your care and help ensure you get the treatment you need.
  • Tobacco cessation program. Quit for Life® gives you the help you need to kick tobacco for good with nicotine replacement therapy and one-on-one phone support.
