Welcome Legacy Health employees! Find information on this page about your Legacy Health medical and flexible spending account plans, as well as programs and services included with your coverage.

Find a Legacy + Network Provider: Portland Metro and Silverton Network Areas

Your medical plan covers eligible services and supplies from Legacy + Network facilities and providers for enrolled employees and dependents. Coverage outside the Legacy + Network is limited to:   

  • Emergency and urgent care services for non-routine care (See “Important medical plan terms” in your Employee Benefits Guide.) 
  • Covered services not available through a Legacy + Network provider that are medically necessary (Prior authorization from PacificSource is required before services are rendered.)
  • Covered services received from any licensed chiropractor, acupuncturist, or massage therapist, as well as diabetic supplies 

All other services rendered by a non-Legacy + Network facility or provider for any enrolled employee and or dependent will not be covered with the following exception. Enrolled employees who work in Corvallis, Eugene/Springfield, or Newport and their enrolled dependents will be covered. Please be sure to review the Legacy + Network provider list before scheduling healthcare appointments. See “Summary of Medical Benefits” in your Employee Benefits Guide for more information.

Important notice for employees who work in Eugene, Springfield, Corvallis, or Newport: Only providers listed on the new city-specific network directory (see links below) and Legacy + Network providers will be considered in-network for you and your covered dependents. 

Your medical plan covers eligible services and supplies from in-network facilities and providers for enrolled employees and dependents. Coverage outside the network is limited to:

  • Covered services not available through a Legacy + Network provider that are medically necessary (Prior authorization from PacificSource is required before services are rendered.)

All other services rendered by an out-of-network facility or provider for any enrolled employee and or dependent will not be covered. Temporary ID Card 

Once your PacificSource coverage begins, you will need to present your member ID card when you visit your doctor or pharmacy to ensure they have the correct insurance information. Print a temporary ID card now.

Register for InTouch, and go paper free!

Once your coverage begins, you can register for InTouch for Members, our secure member website. InTouch gives you 24-hour access to plan materials and benefits, such as your out-of-pocket maximum or limits, Explanation of Benefits (EOBs), and the status of authorizations and referrals.

You can even sign up to go paperless and receive EOBs by email. Keep in mind it may take a few weeks before your benefit summary and member handbook are available through InTouch.
