Our Vision

  • To support and assist charitable programs and initiatives that address the healthcare needs of children and youth, as well as adults who have barriers to care, in communities served by PacificSource Health Plans.
  • Foundation 2To advance and promote strategies that:
    1. Improve access to high quality healthcare,
    2. Test and implement innovative care models,  
    3. Improve community health, and/or
    4. Lower costs across the system.
  • To work in partnership with PacificSource Health Plans and its many business partners and customers, including physicians and other healthcare providers, to improve community health.

Our Community Investment Approach:

  • Responsive grants for safety net provider organizations, providing healthcare services to the underserved with an emphasis on children and youth.

  • Strategic grants supporting initiatives aligned with our mission.

  • Catalytic investments aimed at specific health-related issues, characterized by diverse community collaboration and investments over time.

  • Technical Assistance and Convening around promising models, areas of shared learning and collaborative funding opportunities.
