Support for Members with Diabetes

If you have diabetes, it’s important to stay on top of your condition and keep blood sugar levels from going too high or too low. Diabetes can cause serious health problems including heart disease, blindness, kidney failure, problems with feet/legs, and circulation problems.

Having a chronic condition can be challenging, but taking care of yourself and working with your doctor can help prevent future problems.

What Our Diabetes Support Program Offers 

  • A nurse and a dietitian to answer your health questions 
  • Personal support from a health coach to help you reach your health and wellness goals 
  • Ongoing support to help you maintain healthy lifestyle changes 
  • Newsletters with current and helpful information about your health condition

There is no cost for the program for eligible PacificSource members.
