Community Health Excellence

PacificSource Program Promotes High Quality Care for All

Playing a role in patient care improvements and having a positive impact on community health is important to us here at PacificSource. So we’re excited about a new program called Community Health Excellence (CHE), which we launched in early 2009.

Through this program, PacificSource makes financial and other contributions toward initiatives that are independently evaluated as having a significant impact for patients, regardless of their insurance status.

In addition to meeting certain basic criteria, providers must: 

  1. submit an application and detailed budget,
  2. demonstrate clear objectives supported by measurable data to evaluate progress toward the project’s goals and expectations, and
  3. ensure the proposal will benefit all the provider’s patients—not just PacificSource members.

2009 CHE Program Recipients

The following seven medical groups and hospitals joined PacificSource in the initiative and have begun a variety of quality and patient care improvements throughout Oregon and Idaho. See more details about the projects.

  • Women’s Care – Eugene
  • Oregon Medical Group – Eugene
  • St. Charles Medical Center – Bend
  • High Lakes Health Care – Bend
  • Treasure Valley Pediatrics – Boise

How to Get Involved

We are looking for new provider partners and new patient care improvement ideas for 2010. If you are interested, we invite you to download and complete the 2010 Community Health Excellence Guidelines and Application and the CHE Itemized Budget Form.
