Oregon Group Health Plans
PacificSource offers a wide variety of group medical and dental plans to Oregon businesses.
Who qualifies for group coverage?
State insurance law regulates what types of organizations qualify as a "group" for insurance purposes. In general, to qualify for group insurance coverage a business must have two or more employees who work at least 17-1/2 hours a week; some other conditions apply. What types of group plans are available?
Medical plans
All PacificSource group health plans include benefits for preventive care, physician office visits, and hospital services. Optional coverage is available for prescription drugs, chiropractic care, acupuncture and naturopathic treatment, and vision care. Our plans also come with extra perks at no additional cost. We offer a wide variety of medical plan designs within each of the following product lines.
- Preferred PPO plans offer members the widest choice of providers with few restrictions on provider access. Reimbursement is based on a fee schedule and many combinations of deductibles, copayments, and coinsurance are available. Some Preferred plans can be paired with an HSA; learn more about consumer-directed health plans.
- Prime managed care plans are similar to HMO-style coverage. Members choose a primary care physician (PCP) from the plan's provider panel to coordinate their care. For treatment by specialists, members must have a referral from their PCP to get the highest level of coverage.
- Choice open access plans blend the benefits of our Preferred and Prime products. Members still select a PCP to coordinate their care, but they can access care from other providers without a referral.

Dental plans
We offer a range of group dental plans with annual deductibles ranging from zero to $100 per person. All plans cover preventive and diagnostic services, basic dental procedures, and major treatments. Our dental plans don't rely on a network of participating dentists; instead, members may visit any dentist, and we cover reasonable and customary charges for the area where services are rendered.
Businesses with two or more employees may purchase a PacificSource dental policy to enhance their PacificSource group medical coverage.
Stand-alone dental coverage (without accompanying PacificSource medical coverage) is available to businesses with ten or more employees.
- Businesses with at least 26 employees may add optional orthodontia coverage to their PacificSource dental plan.
