AccordantCare^™ Rare Disease Program

PacificSource AccordantCare^™provides a personal healthcare support system to our members with certain chronic health conditions. This program is designed to help meet the unique healthcare needs of members with:

Amyotrophic lateral sclerosis (ALS)

Chronic inflammatory demyelinating polyradiculoneuropathy (CIDP)

Cystic fibrosis  

Dermatomyositis

Gaucher disease

Hemophilia

Multiple sclerosis

Myasthenia gravis

Parkinson’s disease

Polymyositis

Rheumatoid arthritis

Scleroderma

Seizure disorders

Sickle cell disease

Systemic lupus erythematosus (SLE)

The PacificSource AccordantCare program focuses on helping people with these conditions identify common symptoms or concerns, monitor changes in their health, gain a better understanding of preventive health strategies, and coordinate healthcare services with their medical providers. The program offers:

• Specialized support. Specially trained AccordantCare nurses are available by telephone 24 hours a day, seven days a week, to answer medical questions and assist with condition-related concerns.

• Monthly newsletters with preventive tips and other useful information.

• Access to special members-only features at www.Accordant.com. The site features a rich archive of library articles and drug information, and AccordantCare members can participate in interactive health and wellness programs.
