Condition Management Program
Our Condition Management Program provides personalized support through educational materials and one-on-one communication with knowledgeable nurses. Members with conditions such as asthma, diabetes, heart failure (HF), chronic obstructive pulmonary disease (COPD), coronary artery disease (CAD), or juvenile diabetes are referred to the program.

Does my plan include Condition Management?
Condition Management is available to members insured by large employer groups (those with 51 or more employees), and members on portability policies. If you're not sure whether or not you qualify, check with your company's benefits administrator or feel free to contact our Customer Service Department.

Condition Management is designed to help participants improve their health and well-being, as well as control the healthcare expenses associated with costly chronic diseases. The program can help you:

How does the Condition Management Program work?
The Condition Management Program provides condition-specific educational information and toll-free access to a nurse 24 hours a day. Some members also work with a nurse case manager who will help them build a personal action plan to successfully manage their condition. Condition Management supports your relationship with your physician and empowers you to become more in charge of your own health.

Alert Systems
By monitoring your compliance to recommended care or treatment, Condition Management can help you stay on track with your plan of care.
Counseling
Some participants receive personalized counseling and assistance from trained nurses, dieticians, and other medical counselors for issues pertinent to their condition, such as nutrition, medication, and lifestyle issues.
Support Network
If you are working with a nurse care manager, they will help you monitor your condition. Your physician will also be notified if you agreed to participate in the program. You might also be referred to a pharmacist, dietician, or other specialist for further support. In addition, participants are supported by bilingual services, a quarterly newsletter, and an interactive Web site.
