Chronic Disease Self-Management Workshops
You can have a chronic medical condition and still enjoy life! By managing your symptoms, you can still do the things you love. PacificSource Health Plans, in cooperation with local agencies, offers a six-week (six session) workshop to help you achieve your health goals.
