Have You Taken Our Broker Service Survey?

In late July, we e-mailed you an e-survey invitation, asking for your help in assessing our service to brokers. If you’ve not yet done so, we ask that you take five minutes or so to share your opinions with us. Any agents or support staff members who write PacificSource group business are welcome to complete the survey.

One of our key service goals is to consistently meet or exceed your expectations as a business partner. This confidential survey will help us identify strengths and—more importantly—areas PacificSource can improve upon to make it easier for you to do business with us. Flu Shot Posters Inform Employees of New Choices

Flu Shot Clinic posterIn the July 11 Agent Bulletin and in this month's Benefit Matters plan administrator newsletter, we announced the expansion of our flu shot coverage for the 2008 flu season. Beginning this year, PacificSource members can enjoy greater access and less out-of-pocket expense for flu vaccinations by utilizing flu shot clinics at select retail pharmacy chains or their county health department.

We’ve created a new poster to help employers with medical coverage educate their employees about new choices for obtaining flu shots. The poster is the latest addition to our Workplace Wellness Toolkit. The poster, a payroll stuffer, and a newsletter article are available at no cost to groups. Plan administrators can contact their Client Service Representative for these and other workplace wellness materials.

Two Oregon Prescription Drug Plans Phased Out

Rx $100 DED Tiered 10/20/40 SD MAC B and Rx 20% Mac B, two of our Oregon prescription drug plans, will no longer be available for new or renewing groups effective October 1, 2008. We are phasing out these plans due to low enrollment.

Our Tiered 15/30/50 SD Mac B plan will replace the phased-out plans. Small employer groups currently enrolled in either of the phased-out plans will be moved to Tiered 15/30/50 SD Mac B at their renewal. Large employer groups will be allowed to continue their plans upon request. If you have a client who would prefer another prescription drug plan instead of Tiered 15/30/50 SD Mac B, please have them notify us prior to their renewal.

Letters describing the prescription drug plan modification will be included with renewal materials for groups currently on either of these plans.

Forms Substantiate Oregon Small Group Work Hours

If a sole proprietor or partner is unable to provide proof that they regularly work at least 17.5 hours or more per week, PacificSource may ask for one of the following forms:

  • Work Hours Confirmation Form (27 KB PDF). We may require the employer to complete a Work Hours Confirmation Form to certify that they currently and regularly meet the hour requirement. This form may be signed by either the owner or the accountant.
     
  • Oregon Quarterly Form 132 (Employee Detail Report Form).
