Current HSA Application Is Online

We all like to save paper in the Pacific Northwest, so some of your clients may be tempted to use an old Health Savings Account (HSA) application from last year. However, the HSA member applications were updated in the spring, and HSA Bank will only accept the new version. HSA Bank is now required to gather additional information for the primary account holder and the authorized signer.

Please make sure your clients are using the most up-to-date HSA application. For your convenience, we provide this application as part of our HSA Member Enrollment Packet in the For Agents/Materials/Administrative Forms area of our Web site. FlexPerks and HSA member fliers are included to round out the packet. If you would prefer paper copies of these materials for your groups, you’re welcome to contact your PacificSource Sales Representative.

PacificSource Supports Tobacco Cessation

Since November 15 is the American Cancer Society’s Great American Smokeout®, now is a great time to encourage tobacco users to make a healthy lifestyle choice by quitting tobacco. PacificSource supports tobacco cessation through plan benefits and value-added services, including:

  • A Smokeout communication campaign with posters, payroll stuffers, and newsletter articles, is part of our new Workplace Wellness Toolkit, designed for employer groups with 50 to 100 employees. Groups may want to use the campaign to kickoff a workplace wellness program. Our Client Service Representatives will be distributing these toolkits to plan administrators, so if you have a client you think would benefit from this item, please contact us.
     
  • The Free & Clear® Quit For Life™ Program is available at no cost to eligible PacificSource members. Through one-on-one phone sessions with a Quit Coach, Free & Clear helps tobacco users create a personalized quit plan and provides ongoing support throughout the quitting process. Members can call Free & Clear toll-free at (866) QUIT-4-LIFE (784-8454) for more information or to enroll. Free & Clear member fliers (Oregon, Idaho) are available in the For Agents/Materials area of our Web site.
     
  • Online information and assessment tools for members: Members can log on to the secure InTouch for Members area of our Web site to access InTouch With Your Health where they’ll find the Health Manager, an online health and wellness center powered by WebMD®. Members can use the HealthQuotient® health risk assessment tool to learn their level of risk for disease, then access a wealth of highly reputable information to help them make health decisions.
     
  • Hospital-based health education classes: Members and their covered dependents can participate in hospital-based health education classes and programs, and receive reimbursement. Many hospitals offer tobacco cessation programs. Visit our Hospital-Based Health Education Classes page for more information.

An Ounce of Flu Prevention…

…is worth a pound of saltine crackers and missed work.

Although that's not quite how the saying goes, it’s true that a flu shot is an effective way to minimize the chance of catching the flu. Our 2007 flu shot allowances are based on Medicare rates. Below are the guidelines to our coverage for this vaccination:

Employer-Sponsored Flu Shot Clinics (groups of 100+ only)

  • Our Client Service Representatives can arrange coverage for these clinics as long as the group and clinic meet all requirements. Contact your Client Service Representative for more information or to request a Flu Shot Clinic Request Form, which we require prior to consideration.
  • If coverage for the clinic is approved, benefits are paid up to $25 per injection/per employee.

Flu Shots from Participating Providers

  • Benefits are paid under the medical plan’s preventive care/immunization benefit at the participating provider benefit level.
  • Applicable copays and deductibles apply.

Flu Shots from Nonparticipating Providers

  • This category includes retail stores, such as Wal-Mart, Safeway, or Fred Meyer, or other outlets that provide flu shot clinics to the general public.
  • Benefits are paid under the medical plan’s preventive care/immunization benefit at the nonparticipating provider benefit level.
  • Applicable copays and deductibles apply.
