Current HSA Application Is Online We all like to save paper in the Pacific Northwest, so some of your clients may be tempted to use an old Health Savings Account (HSA) application from last year. However, the HSA member applications were updated in the spring, and HSA Bank will only accept the new version. HSA Bank is now required to gather additional information for the primary account holder and the authorized signer. Please make sure your clients are using the most up-to-date HSA application. For your convenience, we provide this application as part of our HSA Member Enrollment Packet in the For Agents/Materials/Administrative Forms area of our Web site. FlexPerks and HSA member fliers are included to round out the packet. If you would prefer paper copies of these materials for your groups, you’re welcome to contact your PacificSource Sales Representative.
