New Contracts Due in April
If you haven't done so yet, please mail your signed contract to us right away. We need to receive it by April 27, 2007, to keep your PacificSource appointment current and to avoid delays in your commission payments. If you have any questions regarding the new contract, you are welcome to contact your PacificSource Marketing Representative. We look forward to continuing a long and rewarding partnership with you.

Where do I mail it?
Send your contract to Candice Wilkins, PacificSource Marketing Department, PO Box 7068, Eugene, OR 97401-2224. Please sign and date the contract in all places indicated. We must receive it by April 27.
My agency already mailed the contract, but I also received a separate copy from PacificSource. Do I need to complete and mail that contract, too?
No. We only need a contract from the agency, not from each agent.
Since I'm already appointed with PacificSource, do I still need to complete the contract?
Yes. The contract and business agreement have been updated and must be returned to us in order for you to retain your PacificSource appointment.
I already have a business associate agreement with you. Do I need to complete it again?
Yes. The agreement has been updated to reflect the current HIPAA requirements.
Do I need to send in another copy of my state health insurance license?
Yes. Please send in another copy so that we have the most current copy on file and to ensure your appointment continues without interruption.
Do I need to send in another copy of the E&O policy or the cover page of the policy?
Yes. Please send in another copy of the E&O declarations pages (not the entire policy) so that we have the most current copy on file and to ensure your appointment continues without interruption.
Do I need to send a new W-9 or direct deposit form?
No, unless you need to make a change. If you do not currently have direct deposit and you want to sign up, then please complete the direct deposit form.
I already signed and mailed my new contract. When is it effective?
If you accepted and returned the new contract, your current contract will terminate April 30, 2007, and the new contract will become effective May 1, 2007. Your period of reappointment will not change under the new contract.
Will I only receive commission for each enrolled employee under the per-member-per-month commission arrangement?
No, you will receive commission for each member, not just each employee. Members include employees (subscribers), covered spouses/partners, and covered children.
- I have a large group with negotiated commissions. Will my commission rate change?
No. If you have negotiated commissions on a large group, we will continue to honor that.

Caremark® and CVS Merge
As you may have heard, Caremark Rx, Inc. and CVS Corporation have officially merged to form CVS/Caremark Corporation. Caremark is our pharmacy benefits management vendor, and Caremark Specialty Pharmacy Services is our provider for injectable medications and biotech drugs. We do not anticipate any change in our service as a result of this merger, and our members can continue to use the existing Rx plan number, toll-free phone number, and Web site to access their pharmacy benefits.

Communicating Our Privacy Policy to Members
Health insurance organizations must notify their members at least once every three years on how their medical information may be used and disclosed. PacificSource meets or exceeds this statutory requirement. We mail the notice to all members with our annual Member Bulletin newsletter each December. The notice is also included in all our Member Benefit Handbooks. Members may also find it on our Web site, www.pacificsource.com, under the link, “Our Privacy Policy.”
